We are giving below clarifications and answers to the most asked questions to the members of the Congress’ organizing committee, plus other helpful information. Please read carefully and register to the Congress before March 1. Many among you have reserved their rooms at the hotel. Time is passing by, so do not forget to register to the Congress too.

 

 

** Register on line for the Congress, Dinner Cruise and Gala Banquet. Please remember to include the 3 digit security code (CVV2) which is given on the back of your credit card, and select one of the following charge cards only: Visa, Master Card or Discover.

(follow the instructions given below, at the end of the English version)

 

**Registration for the Congress has 2 parts. Both parts need to be completed.

            1. The first part is the Registration Form which consists of personal and professional information, registration fees, Dinner Cruise and Gala Banquet, events you may want to sign up for.

            2. The second part is the actual Payment Form 

 

** Registration Fee Clarification: the fee for Pharmacists, Nurses and Allied Health Professionals is $550 for early registration and $625 after March 1, 2009. Please make sure that your payment reflects the appropriate amount. Your cooperation in this matter is appreciated.

 

** To qualify for student Status (free registration), proof of full time student status including undergraduate, nursing, pharmacy  and pre-doctoral medical programs, etc. must be provided when registering. Please e- mail or send a letter from your school’s registrar indicating your full time academic status in good standing as well as a copy of your photo ID. Candidates for post doctoral programs including Masters and PhD qualify for the reduced fees of $400 before 3/1/09 & $475 afterwards.

 

** Spouse registration of $100 includes the Wednesday Welcoming Reception. Spouses who are Health Care Providers and want to participate in the Congress need to register according to their qualifications (Doctors $600 – 675; Nurses, Pharmacists, Allied Health $550 – 625)

 

**Early on-line registrations will be considered completed only after full payment is received by March 1, 2009. If payment is received after March 1, 2009, the higher rate will apply.

 

**The Dinner Cruise is a very popular attraction and many people have already signed up. We urge you to register and pay as soon as possible. Final arrangements as to the size of the vessel will be determined according to demand and we want to accommodate everyone. Our goal is to have “No guest left behind”

 

**Wired Funds: Some of our international colleagues prefer to make wire transfer payments. Please e-mail us the total amount to be transferred and the names of the registrants so that we can have an accurate record.

**Hotel Reservations: Please make you room reservations as soon as possible in order to take advantage of the special room rates of $255 plus tax. The event code is AAH. Please note that this rate will be honoured for the two days proceeding as well as following the meeting. After March 1, 2009, the special negotiated rate is subject to limited availability. The standard hotel rate is $350 plus tax.

Nous vous donnons plus bas des clarifications et des réponses aux nombreuses questions qui ont été posées aux membres du comité organisateur du Congrès, ainsi que d’autres informations utiles. Veuillez lire attentivement et inscrivez-vous au Congrès avant le 1er mars 2009. Beaucoup parmi vous ont déjà réservé leurs chambres d’hôtel. Le temps passe et il ne faut pas oublier de vous inscrire au Congrès aussi.

 

** Inscrivez-vous en ligne au Congrès, au Dîner-Croisière et au Dîner de Gala. Rappelez-vous qu’il faut inclure votre code de sécurité digital (CVV2) qui se trouve à l’endos de votre carte de crédit et choisissez seulement une carte des marques suivantes : Visa, Master Card et Discover. (Suivez les instructions données à la fin de l’INFO).

 

**L’inscription au Congrès a deux sections. Les deux sections doivent être remplies.                                 

1.                      La première est le Formulaire d’Inscription qui comprend des rubriques d’informations d’ordre personnelle et professionnelle, les fraîs d’inscription, le Dîner-Croisière, et le Banquet de Gala, c.à.d. les événements auxquels vous voudriez vous inscrire.

2.                  La seconde contient le Formulaire de paiement.

 

**Clarification sur les frais d’inscription : pour les pharmaciens, les infirmiers (es) et autres professionnels de la santé,  le montant est de 550 dollars US avant le 1er mars et de 625$ US après le 1er mars 2009. Assurez-vous que vous choisissez la bonne catégorie; votre coopération est très appréciée.

 

**Pour pouvoir prendre avantage des tarifs étudiants (pas de frais d’inscription) il faut procurer les documents nécessaires faisant preuve de votre statut d’étudiant à temps plein, en soins infirmiers, en pharmacie et en programmes pré-médicaux. Veuillez envoyer par courriel ou par la poste une lettre de votre université, indiquant votre statut à temps plein, ainsi qu’une photo, format passeport. Ceux parmi vous qui seraient inscrits au Ph.D. (diplôme de doctorat) ou en Maîtrise, vous pouvez profiter d’un tarif réduit de 400 $US jusqu’au 1er mars et de 475$ US après le 1er mars 2009.

 

**L’inscription des épouses est de 100 $US et ce montant inclut la réception de Bienvenue de mercredi. Les épouses qui sont des professionnels de la santé et qui voudraient participer au Congrès, doivent s’inscrire selon leurs professions (600 $ pour les médecins (675 après la date d’échéance), 550$ (625$ après le 1er mars) pour les pharmaciens (nes), les infirmiers (ères) etc.

 

**Les inscriptions en ligne ne seront considérées complètes que si votre paiement est reçu avant le 1er mars 2009. S’il est reçu après le 1er mars 2009, le taux non préférentiel sera appliqué.

 

**Le Dîner-Croisière  est un événement apprécié et populaire et beaucoup se sont déjà inscrits. Nous vous prions de vous inscrire aussitôt que possible. Les derniers arrangements pour le choix de la taille du navire ne peuvent être conclus qu’en fonction de la demande et nous voulons accommoder tout notre monde. Notre moto est de ne laisser personne à terre.

 

**Les paiements par transferts : Certains de nos  collègues à l’étranger voudraient faire des paiements par transferts monétaires. Veuillez nous envoyer par courriel le montant que vous voulez transférer et les noms de ceux qui s’inscrivent, afin d’avoir les données précises et complètes.

**Les réservations des chambres d’hôtel : il faut  réserver les chambres d’hôtel avant le 1er mars 2009 pour profiter du tarif spécial de 255$ US (plus taxes). Le code à utiliser est AAH. Veuillez noter que ce tarif s’applique pour les deux jours précédents le Congrès et les deux jours suivants le Congrès. Après le 1er mars 2009, ce tarif spécial est sujet aux disponibilités des chambres. La nuitée standard de l’hôtel est de 350 $ US.
